Social Worker - Hospice and Bereavement Support

Position Overview
As a Hospice Social Worker at Pallatus Healthcare, you will provide crucial psychosocial support to patients and families facing end-of-life challenges. You'll work as part of our interdisciplinary team to address emotional, social, and practical needs while helping families access community resources and navigate complex healthcare decisions.
Key Responsibilities
- Conduct psychosocial assessments and develop care plans addressing emotional and social needs
- Provide individual and family counseling to help cope with illness, loss, and grief
- Facilitate advance directive discussions and end-of-life planning conversations
- Connect families with community resources, financial assistance, and support services
- Lead grief support groups and provide bereavement follow-up services
- Collaborate with interdisciplinary team to ensure comprehensive patient care
- Document patient interactions and maintain compliance with regulatory standards
- Provide crisis intervention and emergency psychosocial support
- Educate families about the dying process and what to expect
- Coordinate with nursing homes, hospitals, and other care facilities
Required Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- Current state licensure (LCSW, LMSW, or equivalent)
- Minimum 2 years of clinical social work experience
- Experience with grief counseling and bereavement support
- Strong assessment and crisis intervention skills
- Excellent communication and active listening abilities
- Cultural sensitivity and ability to work with diverse populations
- Valid driver's license and reliable transportation for home visits
- Computer proficiency and electronic health record experience
Preferred Qualifications
- Previous hospice, palliative care, or healthcare social work experience
- Certification in thanatology or grief counseling
- Experience facilitating support groups
- Bilingual capabilities (Spanish preferred)
- Knowledge of Medicare/Medicaid and insurance processes
- Background in family systems therapy or trauma-informed care
- Experience with advance care planning and end-of-life decision making
