Physician - Hospice and Palliative Care

Position Overview
As a Hospice and Palliative Care Physician at Pallatus Healthcare, you will provide compassionate medical care to patients with life-limiting illnesses. You'll work collaboratively with our interdisciplinary team to develop and implement individualized care plans that prioritize comfort, dignity, and quality of life for patients and their families.
Key Responsibilities
- Conduct comprehensive medical assessments and develop personalized care plans for hospice patients
- Manage complex pain and symptom control protocols with expertise in palliative medicine
- Lead interdisciplinary team meetings and coordinate care with nurses, social workers, chaplains, and other specialists
- Provide on-call coverage and respond to urgent patient needs in home and facility settings
- Communicate compassionately with patients and families about prognosis, care options, and end-of-life planning
- Document patient encounters thoroughly and maintain compliance with regulatory standards
- Mentor and educate team members, patients, and families on hospice and palliative care principles
- Participate in quality improvement initiatives and community outreach programs
Required Qualifications
- Medical degree (MD or DO) from an accredited institution
- Board certification in Internal Medicine, Family Medicine, or related specialty
- Valid state medical license and DEA registration
- Minimum 2-3 years of clinical experience, preferably in hospice, palliative care, or related field
- Strong communication and interpersonal skills with ability to discuss sensitive topics compassionately
- Commitment to interdisciplinary team-based care approach
- Electronic health record (EHR) proficiency
- Valid driver's license and reliable transportation for home visits
Preferred Qualifications
- Board certification or fellowship training in Hospice and Palliative Medicine
- Previous hospice or palliative care experience
- Experience with pain management and complex symptom control
- Bilingual capabilities (Spanish preferred)
- Experience with Medicare/Medicaid documentation and compliance
- Leadership experience in healthcare settings
